The Add / Edit Asilomar screen is accessed via Admin > Admin Options
To use the Asilomar Report found in Access Reports you will need to first map the Subtypes in your database to the proper section in the Asilomar Report format.
How to Map your Subtypes to the Asilomar Report
To setup your Asilomar report, go to Admin Options - Add/Edit Asilomar.
In the operation type dropdown you will have your choices of Intakes and Outcomes. In the screen shown above it will default to the owner surrender when you first come to this screen so we will use this as our example.
Under the Potential Subtypes section these are the actual subtypes you use in your Intakes and outcomes that you have defined in your system. Your going to pick one of them, for this example we will use Cremation.
Once selected you will notice that the second dropdown located above the submit button will become activated. This dropdown will give you a list of possible mapping choices for that particular subtype.
For an example the Operation Subtype 'Cremation' is mapped to the Asilomar report equivalent of Euthanasia Request.

Make sure to click submit after you have mapped each Operation Type.
